as a intro this happened for years since i start playing from 2018 october to now!, when i go offline in the middle of a adventure , the next time i go online it will say you died!! with no reason i died! almost each time this happened! so everyone will die after going offline? is this a rule or a bug just to ask?
and then some of my precious belongings are lost after this death like diamond chestplate and elytra

2. In which game mode you encounter this problem?
3. Is this happening each time you log off the game, or only when you log off out of your base/home?

I think this usually happens when you don't exit the game properly, so the game still thinks that you are still in the world and then a mob kills you.
on mac it happens frequently
no no no not, it let you die with no reason, instead of having a label at the top saying that you have been killed by any mob, it said that : " you died!"
Did you fully log out as in menu first then shut the game down because if you don’t menu before you quit the game your entity will be delayed as this was the counter measurement for people abusing the force quit (fully cutting off network to the game instantly makes your entity disappear)
I get the message you died If the and get's really laggy so It might just be a mob that It can't properly identify because of the lag.

I played 3 times and every time I come back... Im either in a wall and die or I have lost ma previous logged in game progress and nothing works properly....
